Ongoing Services Case Manager (Spanish-Speaking)
Description

Under the direction of the Ongoing Services Manager, the Ongoing Services Case Manager provides direct services to refugee, Cuban-Haitian entrants, Afghan and Ukrainian parolee clients, and Special Immigrant Visa (SIV) holders, focusing on those in their first five years in the US.  The Ongoing Services Case Manager assists clients with Information & Referral services to support clients’ access to community resources and public benefits.  Through individual/family case management and workshops, the Ongoing Services Case Manager provides Social Adjustment services including health, home management, and crisis intervention, with the aim to increase clients’ self-sufficiency. The position will involve frequent in-person work in the field.  The Ongoing Services Case Manager should be available to work both remotely and in the NewAP office, following NewAP policy regarding office re-opening. Spanish language fluency, written and spoken, is required for this position.

Responsibilities
  • Recruits eligible refugee, Cuban and Haitian entrants, Afghan and Ukrainian parolees, and SIV clients for workshops, Social Adjustment, and Information & Referral services
  • Provides Information & Referral and Social Adjustment case management services for clients
  • Assists clients to apply for community resources and public benefits for which they may be eligible, including Food Stamps, Medicaid, Refugee Cash Assistance, TANF, WIC, Energy Assistance, etc.
  • Schedules appointments for clients, ensuring transportation and interpretation are available
  • Provides one-on-one education, advocacy, and case management in response to client needs, for home management and crisis situations
  • Accompanies clients to appointments and partner agencies as needed for services (e.g. medical facilities, public benefits office, schools, courthouse, driver’s license office, library, etc.)
  • Organizes regular Social Adjustment workshops on topics of interest to clients, recruits speakers to present at workshops
  • Builds and maintains relationships with partner agencies to ensure effective referral pathways and increase refugee clients’ access to community resources/programs
  • Completes Family Self-Sufficiency Plans and Individual Work Plans for Social Adjustment clients
  • Maintains electronic case files and ensures proper case notes and documentation for each service, ensures information is up to date and accurate
  • Collects data from other staff on I&R and Social Adjustment services provided
  • Monitors service provision goals and helps staff to reach targets
  • Assists with billing for DHS services when needed
  • Assists in preparing monthly, quarterly and semi-annual reports to funders and internally
  • Supports other case management activities in R & RN Department, including services for new arrivals, cultural orientation, shopping for groceries and basic needs, transportation of clients
  • Provides language interpretation and translation across departments as needed
  • Collaborates with other case managers, employment team, AmeriCorps service members, volunteers, and other staff for service provision
  • Follows NewAP, PRM, and CDC guidance for safe(r) provision of in-person services and in limited in-office work hours, remains up-to-date on latest recommendations
  • Attends meetings and training/planning groups as assigned
  • Works occasional evenings and weekends as required
  • Other duties as assigned
Qualifications
    • A personal passion to assist refugees and immigrants
    • Spanish language fluence, spoken and written, is required.
    • Ability to handle volume of detail involving confidential information and requiring execution of assignments without close supervision
    • Flexibility and ability to problem solve
    • Strong interpersonal skills
    • Ability to recognize and be sensitive to cultural, ethnic and religious issues
    • Excellent skills in teamwork and organizational skills
    • Ability to balance priorities effectively while working individually or as part of a team

    • Ability to effectively work remotely and utilize technology platforms including Zoom, Microsoft Teams, and Salesforce
    • Scheduling flexibility required as occasional evening/weekend client assistance is needed
    • Driver’s license, good driving record, and access to reliable personal vehicle required. This position involves frequent use of agency and personal vehicles to transport clients/field work.
    • All drivers must maintain coverage levels on their personal automobile liability insurance of $50,000/$100,000 Bodily Injury and $50,000 Property Damage.
Job Benefits

This is a full-time, salaried position with an annual salary range of $38,500 - $44,500. Employer contributes to medical, dental and vision insurance. Employer pays life and disability insurance. 14 paid holidays and a generous paid time off policy. Employee has the option to contribute to 401(k).
